How to correct a signed activity record

SUMMARY

Use this guide to correct a signed activity record without overwriting the original. Corrections create a new auditable version with a required correction reason.

 

 

WHEN TO USE THIS

  • Use this guide if:
    • A completed signed activity contains incorrect information
    • You need to update a record while keeping the original intact
    • You need a documented correction trail for audit purposes

PREREQUISITES

  • Before starting:
    • You have permission to edit completed activities
    • The completed activity exists in Tracking, Reporting, or the stop registration flow
    • If e-signature is required, you have a valid operator identity and PIN

STEPS

  1. Open the completed activity from one of these places:
    • Tracking page
    • Reporting tab
    • Stop registration flow
  2. Click Edit
  3. Wait for the latest version of the activity to open in the editing dialog
  4. Edit the incorrect fields
  5. Enter the correction reason
  6. If e-signature is required, click Continue
  7. Select your identity
  8. Enter your 6-digit PIN
  9. Click Sign

      If e-signature is not required:
      10. Click Save
 

EXPECTED RESULT

  • After completing these steps:
    • A new version of the activity is created
    • The original record remains unchanged
    • The correction appears as the latest version
    • The full version chain remains auditable

TROUBLESHOOTING

Issue: Cannot save or sign the correction

  • Try: Confirm that a correction reason has been entered
  • Try: Check that all required fields are completed

Issue: E-signature modal does not open

  • Try: Confirm the activity requires e-signature
  • Try: Reopen the activity and click Continue again
